Candidate Requirements

HHA Certificate or CNA License
Fingerprints Level II
Valid Driver's License
Secondary ID (Social Security Card is acceptable)
Car Insurance (must have a car)
Current CPR Card
Physical (within the last 6 months). Must state 'Free of Communicable Diseases.'
Negative TB Skin Test (PPD) or TB Blood Test.

• If you've tested positive for TB, then a Chest X-Ray is required.
• A Chest X-Ray Radiology Report must be provided on a separate page.
• Chest X-Rays will not be accepted if it does not state on the report or on your physical that you have previously tested positive on a TB Skin Test or a TB Blood Test.

CEU's completed (within the last 12 months):

• Alzheimer's Disease
• Assistance with Self-Medication
• Domestic Violence
• HIPAA
• HIV/Aids
• OSHA

Copy of a Blank/Voided Check or Direct Deposit Form from your Bank.
